Fluoroquinolone agents are especially effective in patients with severe BC associated with Pseudomonas or Staphylococcus aureus that involves or threatens the cornea. There are no controlled trials published comparing the fourth generation fluoroquinolones, gatifloxacin and moxifloxacin, to any of the older fluoroquinolone agents, or for that matter, any other currently available ophthalmic antibiotic preparations. Any potential differences in efficacy can only be presumed based on differences in bacterial spectrum. Overuse of any antibiotic agent is a notable cause of the emergence of resistance. To date, the paucity of well-controlled direct comparative trials of the fourth generation fluoroquinolones versus other agents makes it difficult to extrapolate theoretical advantages to true clinical superiority. Corticosteroid combination products are often utilized to treat patients with bacterial keratitis, but carry an additional risk that prolonged use may result in ocular hypertension glaucoma with damage to the optic nerve, defects in visual acuity and fields of vision, and in posterior subcapsular cataract formation. Administered alone, topical corticosteroids can increase the risk of infectious complications affecting the cornea, but may or may not have this effect during concomitant antibacterial therapy. Corticosteroid combinations should be used cautiously in unresponsive or undiagnosed ocular infections as they may exacerbate untreated or resistant bacterial, viral and fungal infections, which could result in irreversible visual consequences. Inappropriate use of a topical corticosteroid in the presence of corneal infections can predispose to ocular morbidity, and the use of a topical corticosteroid before the diagnosis of bacterial keratitis can predispose to ulcerative keratitis in eyes with preexisting corneal disease. There are dangers of unmonitored topical corticosteroid therapy for traumatized eyes. Due to the potential risk with corticosteroid combinations, it may be appropriate to reserve their use.
