Burning Mouth Syndrome remains a fascinating, though poorly understood, condition in the field of oral medicine. New evidence for the neuropathic basis of this syndrome is emerging. Synopsis The results of a 5-year randomised study comparing these two approaches to treating menorrhagia are published in JAMA. In this study 236 women referred to hospital clinics in Finland with menorrhagia were randomly assigned to treatment with a levonorgestrel-releasing IUD or hysterectomy and followed up for 5 years. The main outcomes assessed were quality of life using 5-Dimensional EuroQol and RAND 36-item short-form health survey ; , other measures of psychosocial well-being and costs. After 5 years it was shown that the two groups did not differ substantially in terms of health related quality of life or psychosocial well-being. It was also shown that although 42% of women randomised to IUD eventually underwent hysterectomy, costs both direct and indirect ; were substantially lower than in the hysterectomy group US$2817 vs $4660 ; . Satisfaction with treatment was similar in both groups. It is concluded that provision of levonorgestrel-releasing IUD may offer a wider availability of choices for the patient with menorrhagia and may decrease costs due to intervention involving surgery.

Endometrial luminal epithelium has an important role on blastocyst adhesion and subsequent implantation. The aim of the present study is to evaluate the effect of EMF on endometrial luminal epithelium. For this purpose, 30 Wistar rats were divided in to control and experimental groups. Rats in experimental group are exposed to 5mT EMF, produced by 50 HZ alternative current, 4 hours day for 3 months. After the experimental period, uteri were disected apart and prepared for light microscopic studies. According to histological features, endometria in secretory phase, was only selected for morphologic and morphometric studies. In morphometric studies, Vv of nucleus to cytoplasm, mean diameter of nuclei and nuclear axial ratio were determined using photoshop software and point counting method. Statistical analysis of data was performed using Student t-test. The results showed that endometrial luminal epithelium in control group appeared high columnar and their length was 1.230.02 mm, while in experimental group, they looked high cobuidal with highly condensed nuclei and their height was 0.980.05 mm. The difference between two groups was significant p 0.001 ; . Mean diameter or profile of the nuclei in control group was 0.980.02 and in experimental group was 0.780.18 and the difference was significant p 0.01 ; . Volume fraction Vv ; of nucleus to cytoplasm in experimental group was 1.120.15 which was significantly lower p 0.04 ; . than in controls 1.370.10 ; . B-type brain ; natriuretic peptide BNP ; is a neurohormone that is secreted in response to volume expansion and pressure overload of cardiac ventricles. Two tests measuring BNP in plasma have been developed , an enzyme-linked immunosorbent assay ELISA ; and a radioimmunosorbent assay RIA ; . The ELISA test is a bedside test and would be particularly suitable to assist rapid diagnosis on site in primary an emergency care setttings. However, it is 5 times more expensive than the RIA test. In the first paper in this journal, an overview of 19 studies involving over 9000 patients concludes that ELISA and RIA are both accurate but the advantages of rapid ELISA test need to be balanced against the higher cost. The other paper reports on a prospective randomised study that demonstrates BNP testing is cost-effective in the management of dyspnoea in the emergency department. Most data concerning the conversion from normal aging to AD come from small samples selected in various memory clinics, which are primarily attended by elderly people complaining of memory deficits.
