INTRODUCTION Genistein, a well known isoflavonoid, belongs to phytoestrogens, compounds that demonstrate oestrogenic activity. Its oestrogenic potency is significant, especially for oestrogen receptor . Soya, the richest source of this isoflavonoid, may contain up to 250 g of genistein per gram of soya protein Dixon and Ferreira, 2002 ; . High concentrations of genistein appear in the urine after consumption of a soya-rich diet King et al., 1996 ; . The consumption of isoflavone-rich products can account for the numerous biological effects attributed to oestrogens. Shutt and Cox 1972 ; discovered that reproductive disorders and infertility in sheep consuming clover are evoked by two isoflavones, genistein and daidzein. Since that time, many laboratories have investigated the influence of dietary isoflavones on reproductive processes in animals in relation to their oestrogenic potency. Currently, it is acknowledged that genistein influences many physiological and biochemical processes, moreover, its action also manifests in an oestrogenindependent manner Szkudelska et al., 2003 ; . Many of the effects exerted by genistein are considered to be health protective and are well documented in the literature. Therefore, products containing large amounts of genistein are desirable dietary components. Recently, several papers were published concerning the effects of genistein in very young organisms. It was demonstrated that the fetus and neonate are sensitive to oestrogen exposure Faber and Hughes, 1993; Nagao et al., 2001 ; . Genistein, as a phytoestrogen, may also evoke oestrogen-related changes in developing organisms. Prenatal and neonatal exposure to this compound can affect sexual differentiation. The prenatal treatment of female rats with genistein decreases birth weights, reduced the anogenital distance and delays the onset of vaginal openings. Moreover, genisteintreated females have delayed puberty Levy et al., 1995 ; . Subcutaneous injections of a small 10 g ; dose of genistein to rat neonates caused increased pituitary response to gonadotropin releasing hormone, while treatment with high doses 500-1000 g ; of this compound resulted in an increased volume of the sexually dimorphic nucleus in the preoptic area of hypothalamus Faber and Hughes, 1993 ; . Oestrus cycle irregularities and fertility disorders in female rats are also consequences of neonatal exposure to genistein Nagao et al., 2001 ; . Hughes et al. 2004 ; demonstrated that dietary genistein can modify the expression of the oestrogen-regulated progesterone receptor in the uterus of sexually mature rats during pregnancy and lactation and, consequently, may have long-term reproductive health consequences. There are also some positive aspects of genistein in young animals. It was found that perinatal exposure to this isoflavonoid may protect against mammary cancer in rats. The anticancerogenic effects of genistein in adults are well known and are considered oestrogen-independent Wang and Kurzer, 1998 and isoniazid.

Aware of the naturally progressive nature of hyperopia and presbyopia, and that this progression will, with time, increase their need for reading glasses. However, they will not lose the visual benefit they gained from the CK procedure. LOOSE LENS TESTING A monovision simulation is performed to assess monovision tolerance and to determine the amount of correction required to finalize the surgical plan. Monovision simulation strategies include a contact lens trial or loose lens testing. The contact lens trial was required during the FDA CK presbyopia clinical study. However, I prefer a loose lens test at the initial evaluation to determine if the patient is a good candidate for CK near vision correction. The loose lens test preference resulted from my experience using holmium laser thermal keratoplasty LTK ; for near vision correction in 2001. I would perform the loose lens test at the initial evaluation to determine if the patient was a potential candidate for LTK monovision treatment. If the patient passed the loose lens test, a contact lens would be fit for the monovision contact lens trial. A large portion of these patients failed the contact lens trial due to 1 ; difficulty handling the lens ie, insertion, removal, cleaning ; , 2 ; ocular irritation from the lens and or dryness, and 3 ; unsatisfactory vision. However, patients who initially passed the loose lens test but subsequently failed the contact lens trial who still requested LTK treatment, adapted to monovision nicely and were satisfied with their result. Because of these findings, I abandoned routine contact lens trials before monovision surgical procedures LTK, LASIK, PRK and CK ; , unless requested by the patient. The loose lens test determines binocular near and distance visual acuity, demonstrates monovision tolerance, clarifies expectations for informed consent, and helps determine the final surgical plan. During the test, loose lenses ranging from 0.75 to 2.00 are placed in front of the non-dominant eye while the patient binocularly views a near vision chart, to achieve a comfortable J3 near acuity. Then with the loose lens still in place the patient binocularly views the distance chart to evaluate acuity and determine tolerance. The patient must maintain excellent binocular distance visual acuity to be considered for unilateral CK in the non-dominant eye. If distance correction in the dominant eye is needed to maintain excellent distance vision, bilateral CK or LASIK may be considered. If a patient cannot find a satisfactory endpoint during the loose lens test, surgery should not be considered. The seventh International Symposium on the "Role of Soy in Health and Disease Prevention" was held at Bangkok in March, 2007, Thailand. The meeting collected about 1, 000 health professionals, soy researchers, and educators from various countries in the world, providing an opportunity to learn of new findings from latest research and also lively discussion with renowned soy experts and research program managers. For a decade, this symposium has covered the health effect of soy beans in various diseases and symptoms such as cardiovascular, cancer, bone and menopause. During this meeting, many researchers reported that soy food containing high quality protein can help to displace less-healthy sources of protein from the diets in developing countries. Also, cholesterol-lowering effects of soy protein or soy food ; enriched with isoflavon aglycone were reported. Truly, oral supplementation of the isoflavones can reduce serum triglyceride and LDL-cholesterol, and also increase HDL-cholesterol significantly, but these effects are related to the level, duration of intake, gender, and initial serum lipid concentrations of individuals, as pointed out by Reynolds et al [1]. It has been well established that the isoflavones themselves can exert direct benefits to postmenopausal women with coronary diseases. In the meeting, it was reported that the ability of isoflavones to inhibit postmenopausal bone loss was examined in more than 30 clinical trials. Though these effects are still controversial, most presentations suggested the benefits for bone health. For cardiovascular diseases, isofkavone was recommended for relief of hot flash symptom. For breast cancer, it was suggested that early exposure to soy might reduce breast cancer risk later in life. Some presentations suggested that isoflavones suppressed the elevation of prostate specific antigen levels in prostate cancer patients [2]. These indicate accumulating reasons for incorporation of soy food into the diet. There were several reports on the safety of isoflavones in human, suggesting no side-effects. In experiments using ovariectomized nude mice, however, isoflavones stimulated the growth of existing estrogen-sensitive breast tumors. More research will be needed to resolve this controversy. Much of presentations on the effectiveness of soy in prevention of chronic diseases did not appear consistent. The concluding remark of this meeting was that health professionals could feel justified in recommending more incorporation of soy food into the diet. Future challenges will be needed to search for scientific evidences to support soy nutritional quality, health benefit potential, and designing its role in the health promotion programs.
